Hospital Health Committee; It is the arbitrating Health Committee established on the basis of the provisions of Inpatient Health Care Facilities Management Regulation' having the authority to determine the health states of individuals in cases where Health Practitioner's Report signed and approved by one medical practitioner or Health Status Report signed and approved by three medical practitioners from the same specialty is invalid due to authorization or a health committee report is stipulated in the legislation. Health Status Report which is obligatory for our patients to have some certain rights is one of the most important services of our delivery of healthcare. Health Committee Service is offered on the ground floor A Section in Oncology Hospital.
Patients who request Health Committee Report must make an appointment (you can call 182 or visit www.mhrs.gov.tr via internet to make appointment)
Application
Patients who request Health Status Report must apply in-person or with their legal guardian together. During application, they must carry a document that Turkish Identification Number is written on. Health Committee doesn't request photo.
Procedure
Patients who apply to Health Committee are registered in health committee desk and referred to the related polyclinics. Applicants with official referral form must be recorded in our incoming document unit where is located in The Administrative Building of The Hospital. When examination procedures are completed in the polyclinics, the requester is called for the health committee and The Health Committee Members takes their final decision about the applicant.
Types of Reports Certified in The Hospital;
1- Health Committee Report for Disabled Adults
2- Health Committee Health Status Report For Adults inflicted by terror, accident and incident (prepared with application with referral)
3. In accordance with the provisions of Income Tax Law (No. 193), prepared reports based on disability discounts (application with referral is required)
4. Determination of Disability, Incapacity to Work and Occupational Loss of Earning Capacity Rate, The State of Premature Ageing, Disability Reporting For The Determination of Occupational Loss of Earning Capacity and The Rating of Disability which hinder people from working. (Application with referral is required)
5- Overseas Treatment Confirmation Reports
6- Reviewing Reports As A Referral Hospital For Verification
7- Turkish National Police Directorate Reports within the scope of General Sanitation Requirements Regulations
8- Turkish Armed Forces, Turkish Gendarmerie General Command and Turkish Coast Guard Command Reports within the scope of Health Capacity Regulations
